SOCCOAST for Coast Guard

Documents required to complete a SOCCOAST agreement:

  1. Form CG-5311 – OER (Officer Evaluation Report) (Warrant Officer applicants)
  2. Copies of training certificates
  3. SMART (Sailor/Marine American Council on Education Registry Transcript ) Transcript. To request a SMART Transcript, use the following link and follow the applicable instructions: navycollege.navy.mil
  4. DD Form 214 (if applicant is retired or separated from active duty)
  5. DD Form 295 - Application for the Evaluation of Learning Experiences During Military Service
  6. Completed Credit for Prior Learning Student Agreement form.
