SOCNAV for Navy

Documents required to complete a SOCNAV agreement:

  1. Copies of training certificates
  2. SMART (Sailor/Marine American Council on Education Registry Transcript ) Transcript. To request a SMART Transcript, use the following link and follow the applicable instructions: navycollege.navy.mil
  3. DD Form 214 (if applicant is retired or separated from active duty)
  4. DD Form 295 - Application for the Evaluation of Learning Experiences During Military Service
  5. NAVPERS 601-4 (Navy Occupation/Training and Awards History)
  6. Completed Credit for Prior Learning Student Agreement form.
