SOCNAV for Navy

Documents required to complete a SOCNAV agreement:

Copies of training certificates
SMART (Sailor/Marine American Council on Education Registry Transcript ) Transcript. To request a SMART Transcript, use the following link and follow the applicable instructions: navycollege.navy.mil
DD Form 214 (if applicant is retired or separated from active duty)
DD Form 295 - Application for the Evaluation of Learning Experiences During Military Service
NAVPERS 601-4 (Navy Occupation/Training and Awards History)
Completed Credit for Prior Learning Student Agreement form.
