Forms needed for Loan Applications

Direct Loan Application LNDF - to be used for Spring only, Spring/Summer
or loans after 11/29/08. 

FFELP Loan Application LNDF - to be used if you need a loan for Fall/Spring
prior to 11/29/08.

Direct Parent Loan (PLUS) Application - to be used for parents of dependent students

Reduce / Incease your original Loan Request - We ask that you determine your loan needs prior to submitting your initial LNDF Application.  There are times when you might want to decrease your loan amount or have mitigating circumstances as to why you would want to increase your loan amount. Please note that when you alter your original request, there is a potential for delays in receiving any refunds.

- Forms needed for Other Business

Professional Judgment Application - This is used if your circumstances in 2008 have changed dramatically from the information that we are using from 2007 to determine your financial aid.  When completing this form, you will need to include all documentation that you are able and call the Enrollment Services Center at 719-502-3000 to schedule an appointment with a financial aid advisor.

Financial Aid Appeal - This form is to be used if you have been suspended from your financial aid.  Do NOT use this form if you are trying to pursue a Tuition Appeal. 

Change of Major - If you are not in a degree-seeking program or an approved certificate program, you will need to complete a Course of Study Change Form.  Print it out and take it to one of the advising centers for completion. Once this form is complete; you will bring a copy to the ESC.
